APPLICATION FOR
A NEW OR REPLACEMENT OF ARRIVAL CARD: I-94
(Due to theft, Loss or Worn Condition)
This request is many times required
when asking for an Extension of Status with
B-1/B-2 or change of status,
and is not available for the original I-94
Arrival Card (given by the immigration officials at the time of entry to
the United States.)
Before presenting your Petition for a new
Arrival Card: I-94, due to the fact that it was lost, stolen or worn out,
you must have duly prepared your form. We
give you that service, and we indicate what you will need, so that

Upon receiving
